Cardiology Billing Services For Every Part Of Every Test.
Cardiology billing depends on who owns the equipment, who supervised, who interpreted and where the patient was. One echo can be billed as a global service or split into technical and professional parts. We sort that out test by test, and keep devices, imaging approvals and hospital procedures on track.
- Global, technical and professional
- Device monitoring cycles
- Imaging authorizations
No real patient or claim data.

Who Bills Which Part?
The same test code is billed three different ways depending on where it was performed and who read it.
| Scenario | Your practice bills | Modifier |
|---|---|---|
| Test on your equipment, read by your cardiologist | The whole service | None (global) |
| Test in the hospital, read by your cardiologist | Interpretation only | 26 |
| Test on your equipment, read by an outside physician | Technical portion only | TC |
Split Into Separate Codes
Cardiovascular stress tests have their own codes for each part, so the right one depends on who supervised, who ran the tracing and who interpreted.
The Drug Is Its Own Line
Myocardial perfusion imaging pairs the imaging code with the stress test codes and a separate line for the radiopharmaceutical, billed by its HCPCS code and dose. Each needs to match what the record shows was used.
Interpretations must be signed before the professional component is billed. An unsigned read holds the claim.
Pacemakers, ICDs And Monitors Bill On A Cycle.
Remote monitoring of implanted devices is billed per period, not per transmission. Getting the period and the split right helps avoid duplicate-billing denials.
Remote Interrogation, Professional
Pacemaker or ICD review, reported once per 90-day period, at the end of the period.
Remote Interrogation, Technical
The device data service for the same 90-day period, billed by whoever provides it.
Interrogation And Programming
In-person interrogations are not reported for the same device during a remote period; programming evaluations follow their own rules.
Remote physiologic monitoring for blood pressure or weight is separate; CPT 2026 added options for fewer days of data and shorter management time.
Approval Is For One Test, Not One Patient.
Many commercial plans route nuclear stress tests, echocardiograms, CT and PET through radiology benefit managers. The approval names a specific test and date range. Switch from a stress echo to a nuclear study, and the approval no longer fits.
- Approved test code matches the test performed
- Date of service is inside the approved window
- Ordering and rendering providers match the request
- Changes on the day: updated approval requested before billing
Full RCM: we request and track approvals. Medical Billing: your team keeps them; we check each claim against them.
Who Was In The Room Matters Too.
Supervision Levels In Your Office
Medicare assigns each diagnostic test a supervision level: general, direct or personal. Stress testing, for example, needs direct supervision, which Medicare now allows through real-time audio-video for many services. The record should show who supervised.
Cath Lab And Hospital Procedures
Catheterizations, interventions and electrophysiology studies are professional claims from hospital records, with the right place of service and any moderate sedation documented separately.

Questions Cardiology Practices Ask.
Please do not send patient records through the website.

Do You Bill Tests Our Physicians Read At The Hospital?
Yes, the professional component with modifier 26, once the interpretation is signed. The hospital bills the technical portion.
How Do You Handle Remote Device Monitoring?
Per 90-day period, at the end of the period, with professional and technical portions split by who provides each. We track each monitoring period to avoid overlaps.
What If The Approved Test Changes On The Day?
An updated approval is needed before the claim goes out. With Full RCM we request it; with Medical Billing your team does.
Do You Bill Radiopharmaceuticals?
Yes, as a separate line by HCPCS code and dose, matched to the nuclear study and the record.
Can You Work Old Unpaid Testing Claims?
Yes, as a separate Old A/R Recovery engagement. We review each claim's history and deadlines first.
