Pain Management Billing Services Level By Level.

Interventional pain claims are judged against coverage policies that set the order of treatment, the relief required and how often a procedure can repeat. We bill each procedure by level, side and guidance, and check the documentation trail before the claim goes out.

Every Step Needs Its Own Proof.

Medicare contractors and many commercial plans set a defined path to radiofrequency ablation. A claim at step four fails if steps one to three are not in the record.

  1. 1Conservative CarePain lasting about three months or more, functional limits and failed conservative care such as therapy or medication, documented.
  2. 2Diagnostic Block 1Medial branch block with the percentage of pain relief documented afterwards.
  3. 3Diagnostic block 2A confirming block. Many policies require significant relief, often 80% or more, from both.
  4. 4Radiofrequency ablationBilled by level and side, with imaging guidance included in the procedure code.
  5. 5Repeat ablationUsually only after sustained relief for months, within a yearly limit per side and region.

Summarized from Medicare facet joint intervention policies. Exact criteria differ by contractor and plan; we check the policy that applies to each claim.

Six Procedures, Six Sets Of Rules.

Epidural Steroid InjectionsCoded by spinal region and approach, interlaminar or transforaminal. Imaging guidance is included in many codes. Policies limit how often they repeat.
Facet Injections And BlocksPer level, with an add-on for each additional level. Bilateral procedures follow payer-specific modifier rules. Imaging guidance is expected.
Radiofrequency AblationPer level and side. Repeat treatment depends on documented duration of relief and yearly limits.
Sacroiliac Joint InjectionsImage-guided injection, with diagnostic intent and response documented when it leads to further treatment.
Trigger Point InjectionsCoded by the number of muscles injected, one or two muscles versus three or more, not by the number of injections.
The Injected DrugSteroids and other drugs are separate lines, billed in the units their HCPCS code defines, from the dose recorded.

Urine Drug Testing, Billed By Necessity.

Drug testing is one of the most audited areas in pain medicine. Each test needs an individual order and a reason tied to that patient, not a standing panel.

Screening Tests

Confirmatory Tests

Medicare codes definitive testing by the number of drug classes tested. The order should say why each class was tested.

Testing every class for every patient at every visit is a common audit finding. We bill what the individual order and record support.

The Packet Payers Ask For.

Commercial plans often require approval for interventional procedures and review the same evidence each time. A complete packet the first time helps reduce back-and-forth.

Full RCM: we request and track approvals. Medical Billing: your team keeps them; we check each claim against the approval and policy.

What a strong request includes
  • Pain scores and functional measures over time
  • Conservative care tried, with dates and results
  • Relevant imaging findings
  • Relief after each prior block, as a percentage and duration
  • Levels and sides that match the procedure planned

Questions Pain Practices Ask.

Please do not send patient records through the website.

Do You Check Coverage Policies Before Billing An Ablation?

Yes. We check that the record shows the required conservative care, diagnostic blocks and documented relief for the policy that applies to that patient's plan.

How Do You Bill Bilateral Injections?

By each payer's rule: some want modifier 50 on one line, others want right and left on separate lines. We follow the payer's current guidance.

Can You Bill Our Drug Testing?

Presumptive testing in your office, yes. Definitive testing depends on whether your practice performs it or sends it to a lab; we confirm before quoting.

What Happens When A Procedure Is Denied?

We check the denial reason against the record and the policy, then correct, gather records or appeal when appropriate. The payer makes the final decision.
